"Interdisciplinary Approach for Better Aesthetic Results"

Creating the precision in terms of the preparation, fit and the aesthetic final outcome needs a serious treatment planning which is different for each case… Designing a new smile consists of many steps which are so very important and if followed right, produce predictable success. The key to aesthetic excellence requires patient communication as well as the communication with the laboratory and the specialists from the beginning of the case till the end.

In today’s dentistry success depends on an interdisciplinary team work. Every member of the team (including the specialists, lab technicians and the patient) should be aware of the treatment planning and the end result from the beginning. Every effort should be spent to bring the teeth to their natural position on the dental arch, hence creating enough space for the restoration to be received so that the aim should be minimum tooth preparation.

After that stage, in order to achieve a very precise and predictable tooth preparation, wax-up, silicon indexes and related techniques such as APR (Aesthetic Pre Recontouring), APT (Aesthetic Pre-evaluative Temporaries) and preparation through the APTs are extremely crucial. This eliminates all the possible mistakes and destruction of the depth cutters that could have been done if the tooth was to be prepared without realizing the aging effects on the enamel (volumetric enamel loss) or their inappropriate teeth positions on the dental arch....

"Challenging Implant Sites - Anterior Maxilla and Posterior Mandible"

Long term studies have demonstrated that partially or completely edentulous jaws can be restored successfully with implant-supported fixed prostheses. However, resorption of the alveolar ridges often presents the surgeon with obstacles for implant placement. And while these obstacles have been well addressed in the posterior maxilla by means of the sinus augmentation surgery, the posterior mandible presents a very unique problem: the presence of the inferior alveolar nerve.  Implant surgery in the edentulous premaxilla is also often challenging because of esthetic, phonetic, and biomechanical considerations. The purpose of this presentation is to discuss various treatment options for anterior maxilla and posterior mandible.

"The use of temporarily cemented Transitional Resin Bonded Bridges (TRBBs) and devitalised/ endodontically treated teeth for pre-implant or pontic site development in the aesthetic zone"

Unfavourable midfacial tissue levels on anterior teeth scheduled for extraction and implant placement in high smile line patients are a common problem in implant practice. Midfacial soft tissue margins that are more apical than the contralateral tooth soft tissue margin can be expected to migrate further apically following tooth extraction. This paper describes a non-surgical pre-extraction extrusion technique, utilising transitional resin bonded bridges (TRBBs) which are temporarily cemented. By decoronating the tooth to be extracted and utilising elastic ligature and an interim post, these teeth can be extruded into the TRBB, over a number of weeks, ahead of the scheduled extraction  without the use of unsightly brackets or arch wires. This technique has the ability to turn a midfacial tissue deficit into a tissue credit ahead of tooth extraction and implant placement, and can give clinicians a distinct advantage in the treatment of these aesthetically challenging cases.

"Ceramic Abutments: State of the Art"

Dental Implants have been utilized for more than 30 years. Dental implant supported restorations are becoming more accepted by patients. The osseointegrated implants cover a continuous enlarging part of today modern practice and esthetics have become an everyday need in current dentistry. Implant restoration in the anterior maxillary aesthetic region often presents challenges to the clinician. Ceramic abutments represent for the moment the most aesthetic solution a clinician can use as a prosthetic restoration. This occurs because a ceramic abutment has the advantage of providing better esthetic results because of light transmission and light reflection through the ceramic material better than any of the titanium or gold abutment systems.

In this presentation the history of the family of ceramic abutments will be discussed as well as the types of ceramic abutments available in implant dentistry. Also the characteristics of the ceramic abutments will be analyzed in detail. Last but not least, research background will be presented and discussed with different types of abutments and methods of testing and evaluation. Finally a variety of clinical cases will be shown.

"Management of the Anterior Extraction Socket: implant placement with immediate provisional restoration"

The clinical techniques described in this presentation enable practitioners to achieve predictable esthetic success using a method that limits the amount of buccal contour change of the extraction site ridge and potentially enhances the thickness of the peri-implant soft tissues coronal to the implant-abutment interface. This approach involves atraumatic tooth removal without flap elevation, and placing a bone graft into the residual gap around an immediate fresh-socket anterior implant with a screw-retained provisional restoration.
